Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It shares clean trial data and ratings using PRECIS-2 permitting multiple and varied meta-epidemiological research studies to examine the effects of treatment across trials with different levels of pragmatism as well as other design features.
Background
Pragmatic trials provide evidence from the real world that can be used to make clinical decisions. This can be accomplished by ensuring that their analysis is based on an intention-to treat method (as described in CONSORT extensions).
Despite these requirements, a number of RCTs with features that defy pragmatism have been incorrectly self-labeled pragmatic and published in journals of all types. This can lead to misleading claims of pragmatism and the usage of the term should be made more uniform. The development of the PRECIS-2 tool, which provides a standard objective assessment of pragmatic features is a great first step.
Methods
In a pragmatic study the aim is to inform clinical or policy decisions by showing how an intervention could be integrated into routine treatment in real-world settings. This differs from explanation trials that test hypotheses regarding the cause-effect connection in idealized situations. In this way, pragmatic trials may have a lower internal validity than studies that explain and are more susceptible to biases in their design as well as analysis and conduct. Despite these limitations, pragmatic trials may be a valuable source of information for decision-making in the context of healthcare.

The PRECIS-2 tool measures the level of pragmatism that is present in an RCT by assessing it on 9 domains that range from 1 (very explicit) to 5 (very pragmatic). Additionally 36% of 89 pragmatic trials discovered by Koppenaal and co. were placebo-controlled or conducted before licensing, and the majority were single-center. Thus, they are not as common and are only pragmatic if their sponsors are tolerant of the lack of blinding in these trials.
A typical feature of pragmatic studies is that researchers attempt to make their findings more meaningful by studying subgroups within the trial sample. This can lead to unbalanced analyses with less statistical power. This increases the chance of omitting or ignoring differences in the primary outcomes. This was the case in the meta-analysis of pragmatic trials because secondary outcomes were not adjusted for covariates' differences at the baseline.
Furthermore, pragmatic studies can pose difficulties in the collection and interpretation of safety data. This is because adverse events are usually self-reported and prone to reporting errors, delays or coding errors. It is crucial to improve the accuracy and quality of outcomes in these trials.
Results
While the definition of pragmatism does not require that all clinical trials are 100% pragmatic There are advantages to including pragmatic components in trials. These include:
Increasing sensitivity to real-world issues, reducing cost and size of the study and allowing the study results to be faster implemented into clinical practice (by including patients from routine care). However, pragmatic trials can also have disadvantages. The right type of heterogeneity, like could allow a study to expand its findings to different settings or patients. However, the wrong type can decrease the sensitivity of the test and thus lessen the power of a trial to detect minor treatment effects.
A variety of studies have attempted to classify pragmatic trials using different definitions and scoring methods. Schwartz and Lellouch1 developed a framework to distinguish between explanatory trials that confirm a physiological or clinical hypothesis and pragmatic trials that aid in the selection of appropriate therapies in the real-world clinical setting. The initial PRECIS tool3 featured similar domains and scales from 1 to 5. Koppenaal et. al10 devised an adaptation of the assessment, known as the Pragmascope, that was easier to use for systematic reviews. They discovered that pragmatic reviews scored higher in most domains, but scored lower in the primary analysis domain.
The difference in the primary analysis domains could be explained by the way that most pragmatic trials analyse data. Some explanatory trials, however, do not. The overall score was lower for pragmatic systematic reviews when the domains of organisation, flexible delivery and follow-up were merged.
It is crucial to keep in mind that a study that is pragmatic does not necessarily mean a low-quality study. In fact, there are an increasing number of clinical trials which use the term "pragmatic" either in their title or abstract (as defined by MEDLINE but which is neither sensitive nor precise). The use of these terms in abstracts and titles could indicate a greater understanding of the importance of pragmatism however, it is not clear if this is manifested in the contents of the articles.
Conclusions
As the importance of evidence from the real world becomes more commonplace, pragmatic trials have gained momentum in research. They are randomized studies that compare real-world alternatives to clinical trials in development. They involve patient populations more closely resembling those treated in regular care. This approach can overcome the limitations of observational research, like the biases associated with the reliance on volunteers and the limited availability and coding variations in national registries.
Pragmatic trials have other advantages, like the ability to use existing data sources, and a greater chance of detecting significant differences than traditional trials. However, pragmatic trials may have some limitations that limit their reliability and generalizability. For example the participation rates in certain trials could be lower than anticipated due to the healthy-volunteer influence and incentives to pay or compete for participants from other research studies (e.g. industry trials). The requirement to recruit participants in a timely manner also limits the sample size and the impact of many practical trials. Practical trials aren't always equipped with controls to ensure that the observed differences aren't caused by biases during the trial.
The authors of the Pragmatic Free Trial Meta identified RCTs that were published between 2022 and 2022 that self-described as pragmatism. The PRECIS-2 tool was employed to determine the degree of pragmatism. It includes areas such as eligibility criteria and flexibility in recruitment as well as adherence to interventions and follow-up. They discovered 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or higher) in at least one of these domains.
